You will be directed to create an account that will enable us to better manage information season to season. The following forms should be uploaded to your Active account a minimum of ten days prior to your child's attendance at CLL:
Immunization Record - Please provide a copy of your child's immunization records regarding the age-specific vaccinations your child has received, along with the corresponding dates of administration. While we do not provide a specific form for this purpose, your healthcare provider should be able to furnish you with a written statement upon request.
Camper Release Form -To ensure the safety of your child, we require a list of pre-approved adults before they are released to any drivers. To provide us with the necessary information, kindly complete our Camper Release form.
Medical Release Form- If your child will be taking medication while at camp, please fill out and submit the Medical Release Form. Otherwise, the form is not required.
Self-Administration Form - If your child is capable of administering emergency medication, we request that you fill out the self-administration form.
